York Healthcare is committed to supporting both caregivers and patients throughout the transition to Personal Care Assistance (PCA) services. Whether you are entering the program independently or as a caregiver–patient pair, the following information will guide you through the process.

- Not certified? YorkHealthcare provides free training and testing.
- Already certified as a PCA or HHA? You might qualify.
- No experience? Take a 40-hour Basic Training Course.
- If certified, show 3+ months of recent experience or formal training (e.g., HHA, nursing school).

- Not certified? York Healthcare provides free training and testing.
- Certified? You may already qualify.
- 3+ months of personal care experience (home or facility) within the last 3 years, or formal training (HHA/nursing school).
- Cannot be the patient’s spouse, parent, son, son-in-law, daughter, or daughter-in-law.
- Must live at a different address from the patient.
Patient Requirements:
- Must be enrolled in a health plan that works with a participating PCA provider.
- Once the caregiver and patient onboarding is complete, we require the authorization from the patient’s health plan to move forward with LHCSA.

Long Term Care







We provide you with constant assistance for your condition.
For those individuals who are in need of round-the-clock maintenance and assistance, we offer Managed Long Term Care program (MLTC) to ensure that your loved one receives the services they need to gain a better quality of life.
To ensure the long-term care system in New York provides quality care management for all, MLTC enrollment plans may be mandatory or voluntary, depending on the client’s circumstances.
MLTC is mandatory for individuals:
- Who are dual eligible. (for both Medicaid and Medicare)
- Aged 21 years and up and need long-term care services for more than 120 days.
- Residing within the counties of NYC, Nassau, Suffolk, and Westchester.
MLTC is voluntary for individuals:
- Eligible for both Medicaid and Medicare.
- Aged 18 to 21 years old and need long-term care services for more than 120 days.
- Assessed as nursing home eligible.
- Who are non-dual eligible and are over the age of 18 and qualified for nursing home services.
You are qualified for MLTC if you:
- Have a chronic condition or disability qualified for nursing home services.
- Are able to stay safely at home at the time of enrollment.
- Meet the age requirement.
- for a PACE organization, the age requirement is 55 years old.
- for most other plans it is 65 years old.
- Live within the service areas.
- Are willing or have to change doctors who will manage your plan.
- Have a payment method accepted by the plan.
- all plans accept Medicaid and some also accept Medicare and Private Pay.
How to Enroll in MLTC:
To begin the enrollment process in a PACE of MLTC plan, you or someone on your behalf will contact the plan provider in your area. A staff representative visits you in your home to assess your condition and needs before you join to:
- Explain the rules and responsibilities of the plan membership.
- Determine your eligibility for the service.
- Give you a copy of the member handbook and provider handbook.
- Assist you in completing the enrollment application and agreement.
- Assist in completing a Medicaid application, if needed.
- Help you choose a care provider suited to your requests.
- Discuss your care plan needs and requirements.
How Does MLTC Affect my Medicaid or Medicare Coverage?
If you’re receiving Medicaid or Medicare, you won’t lose your regular benefits. MLTC helps coordinate your long-term care services, but you can still use your Medicare or Medicaid card for services not covered by the MLTC plan. You continue to receive the same types of care—MLTC just manages some of them.
